Simply connect each of the 8-bit colors to an DA converter. Preferably a 
triple DAC, these are designed for video applications.

Why ??? Since IBM introduced the VGA and XGA the world has been lost. The 
only standard now is that there is no standard. Some resolutions are 
common, mostly because they make nice binary numbers. But that's it. As far 
as the rest is concerned everybody generates his own. Although VESA tries 
to provide some standards, which apparently everybody supports.
Modern video cards and monitors are capable of generating / displaying a 
vast number of resolutions and refresh rates. Even unusual ones (like 
800x600@50Hz for example).

If you really want some number check the vesa website www.vesa.org (I think).
